Application forms are for the October 5, 2017 deadline. Fill out completely, print and mail everything requested together in ONE ENVELOPE by deadline. Include only current items. Checklist is for your information, not to be included in application.
A current and completed application signed and dated. All information requested must be submitted in order to be considered.
A newdated and signed letter stating your background, your plans and goals, and any other information you wish to include.
A current transcript (High School seniors and first semester freshman submit a high school transcript, all others submit the most recent college transcript). Transcripts MUST have the name and address of institution.
FIRST TIME APPLICANTS must submit letters of recommendation from educators, clergy, employers, etc. All letters are kept on file so only new ones are requested.
